Please take a moment and look over this form. This form will ask you some questions about your medical history. If you need to answer YES to any of these questions you are required to print out the full medical pack and have your doctor sign off on it before you will be allowed in the water with us. The release will be required the first night of class. Just know a yes doesn’t mean you cannot take the class you just need your doctor’s approval first.
If none of the medical questions apply to you and your answers are all NO then you do not need to get the medical release form filled out by your doctor.
